There are only a few things in the maintenance manual that says can make this happen and the linkage is the easiest, the rest is internal. It typically happens while under pressure and you can only adjust so much until you grind going into drive.

I have had to adjust my linkage 2 times, but I have never had it pop out of gear. Your dealer needs to make this right IMHO.
